A Stale 'Blocked' Note Was Copied for a Month: AI Agent Handoff Notes Are Not Evidence

An author running an autonomous agent (acrid) across sessions recounts a telling failure: 'key expired, blocked on a new one' sat in the daily summary for about a month while the key was live the whole time. Every session read yesterday's summary and copied the line forward — nobody ran the call until it finally returned 200.
The core insight: a handoff note reads as evidence even when it's just an earlier session repeating itself. The tell was that the line had a note date but no check date.
Changes made and open problems:
- Blockers must now name the command that proved them and when it last ran; a 'blocked' with no re-test in the past day is treated as unknown;
- But stamps can be copied forward too — a forged-looking 'retested today' line is already foreseeable;
- Open questions posed: should negative claims (can't / blocked / expired / down) be re-verified on a schedule or on use? Should handoffs carry evidence or only conclusions?
A practical lesson for anyone running agents across sessions or shifts: unverified negative facts become permanent blockers.
