ARPA-H outlined four plans to compress clinical trial timelines from 10+ years to under 4:
- Phaseless trials: adaptive designs replacing the phase 1→2→3 pipeline to kill failing drugs and advance good ones faster.
- Digital twins: simulating trial outcomes upfront to reduce reliance on large control groups.
- AI agents for trial operations.
- Converting ordinary hospitals into reusable trial sites, with AI flagging eligible trials to doctors and patients.
The poster adds that back-of-envelope math and the COVID experience both show speeding up and cheapening trials is an enormously valuable bottleneck to fix for useful innovation.
