Claude Code v2.1.287 ships Claude Mods, guardrail side-agent plugin and MCP URL prompts

- Claude Mods: plugins can now modify deeper behavior of Claude Code.
- You should know: an opt-in side-agent plugin that flags things you or Claude might miss; enable via /plugin enable cc-plugin-you-should-know@builtin.
- MCP: support for URL prompts from MCP servers (2025-11-25 protocol, e.g. sign-in flows); if an older server breaks, add bareElicitationCapability: true to its config.
- Also adds an n:<text> session-name filter in the agents view, prompttext in the OpenTelemetry userprompt event, a built-in gh api for self-hosted runners, plus fixes for remote-session fast mode, repeatedly waking hooks, and ignored availableModels on Bedrock/Vertex.
