Codex VS Code extension 26.928 update leaves messages vanishing or stuck spinning

An Enterprise user reports that after the Codex VS Code extension updated to 26.928.31416 on Oct 1, 2026, it became intermittently unusable on Windows: normal messages randomly enter a steer-like pending state, spin forever without completing, or disappear entirely after submission, with the UI stuck on a previous turn.
The reporter suspects the conversation/turn state desynchronizes between the VS Code UI and the Codex backend, treating normal submissions as if an active turn still exists. The update also reinitialized local workspace/session state, taking about 10 minutes. The issue is labeled bug/windows-os/extension/session and did not occur before the 26.928 update.
