Claude Code v2.1.286 fixes session loss, cache billing errors and model fallback retries

Claude Code v2.1.286 ships a batch of practical fixes:
- Sessions: --resume/--continue no longer lose every turn after parallel tool calls when the earlier session crashed; API 400 errors from tools/hooks returning non-text values (including in resumed sessions) are fixed.
- Billing: The apps gateway spend meter no longer prices 1-hour prompt cache writes at the cheaper 5-minute rate, and now counts input tokens correctly on streamed turns with server-side tools.
- Resilience: If the API rejects your default model or alias, Claude Code now retries once on a same-tier model; --fallback-model retries run at standard speed.
- Auth: Fixed duplicate login-browser popups on GCP/AWS credential expiry, stale macOS credentials causing "Not logged in" states, and claude auth status inaccuracies.
- UX: Permission prompts show a "2 of 5" counter when requests stack; fullscreen lists gained mouse support with click-to-jump and hover states.
