Claude Code 2.1.286 ships 88 CLI changes including secret-leak fix

Claude Code CLI 2.1.286 is out with 88 changes:
- Counted permission prompts: stacked permission requests now show progress like "2 of 5"
- Model retry: sessions retry the previous model once when the Anthropic API refuses a model, preventing every-turn failures
- Log redaction fix: secrets are masked even when key names contain invisible characters, preventing leaks
Also fixes --resume/--continue losing turns after parallel tool calls, duplicate login-browser popups on GCP/AWS credential expiry, and more.
