Incident Memory Agent Turns Postmortems into Verified Triage Context via Hindsight

The open-source Incident Memory Agent uses Vectorize Hindsight's Node SDK to turn incident postmortems into active triage context. Retention is gated behind human confirmation and records failed approaches (e.g. 'restarted pods, pool saturated again in 90s') to prevent hallucinations from becoming memory. Recall builds queries from service, error, and tags, injecting reranker-scored memories into Groq-hosted gpt-oss-120b for grounded diagnostics.
