The team open-sourced RecallOps, an AI incident investigation system built around persistent agent memory via Hindsight, following an Incident→Recall→Reflect→Investigate→Resolve→Retain workflow. It recalls similar past incidents, tracks failed remediations as negative evidence so the same fix isn't suggested twice, separates observed facts from hypotheses, and requires human approval before production actions.
