The author built an incident-response agent with Hindsight as a persistent memory layer (incident -> recall -> evidence -> investigate -> recommend -> retain) and ran a small held-out eval: without memory 0/5, with memory 0/5. The takeaway: memory retrieval and reasoning are separate problems — finding relevant past incidents doesn't mean the agent can use them correctly. Making retrieved memory visible helps audit relevance and whether similar symptoms are confused with the same root cause. The author argues agent memory needs a feedback/correction loop rather than just storing more incidents.
