kitaru MCP server adds transition failure matrices to trace agent failures across sessions

Inspired by Hugo Bowne and Hamel Husain's discussion of transition failure matrices on Vanishing Pixels, developer strickvl implemented the concept into the kitaruai MCP server (still a prototype). It tracks where your agent fails across multiple sessions, making it easier to spot recurring failure points. A walkthrough video is included.
