Claude API adds inline tool definitions for adding full tools mid-conversation

Anthropic's Claude API now supports inline tool definitions, letting you add full tools mid-conversation instead of predefining everything in the top-level tools array. Key caveat: editing the top-level array mid-conversation changes the prefix, causing thinking blocks to be ignored on new models with preserved thinking enabled — inline tool addition avoids this.
