Agent task complete isn't case closed: who owns the handoff after AI finishes?

A healthcare operations practitioner highlights a blind spot in AI agent workflows: a handoff can look successful in the logs while the customer's problem goes nowhere.
His hypothetical: a practice outsources billing with an AI agent handling calls. A patient disputes a charge; the agent captures details and requests documentation — but the request lands in a shared inbox, the agent's task is marked complete, and nobody accepts responsibility for following up. A week later the patient calls again and explains everything from scratch.
He argues a proper handoff needs three things: a person or team that accepts responsibility (not just receives a notification), visible missing info / next action / deadline for handlers, and a fallback when no one claims the case or a deadline passes. Agent task completion and case closure are two different milestones.
His suggested metrics: cases that stall after transfer, and time spent waiting between teams — not just whether the agent successfully sent the request.
