Hannah Stulberg's team at DoorDash enforces a rule: a feature isn't rolled out until the repository that agents read is updated—not the docs—otherwise the next person gets a three-month-old answer without knowing it. Payoff: a new engineer pulled three-month-old context on a decision in 15 seconds without Hannah in the loop, using 3% of the context window.
The author reflects that his own PM OS (41 skills) made exactly one person faster—himself; team speed is a different problem. His first CLAUDE.md was the classic mistake: stuffing everything in, hitting context limits by the third query. The fix isn't a bigger file but deciding what each agent is allowed to see, with the orchestrator carrying the map.
