FLARE-AI paper proposes standardized flaw reporting system for deployed AI, seeking a full-time operator

Avijit Ghosh (Hugging Face) and colleagues announced FLARE-AI (arXiv:2606.31567), an open-source flaw reporting system for deployed AI, with a live demo — and they're looking for an organization to run a registry of flaw/incident reports full-time.

- Problem: the AI flaw-reporting ecosystem is fragmented — researchers don't know what or where to report, recipients rarely share reports, and reporters duplicate effort while receivers lack triage-ready information.
- Audit: the team audited 12 reporting systems from AI developers, cybersecurity groups, and aggregators, identifying five recurring design challenges: discoverability, scope, information collection, coordination, and guidance for strict-liability cases.
- Method: built on that audit plus feedback from 49 experts across 32 organizations, FLARE-AI is designed for interoperability, collecting triage-relevant info via conditional fields.
An accompanying discussion proposes "model card"-style fields for misalignment incidents: incident date, frequency (% of affected rollouts), stage (eval vs RL training), whether existing monitoring caught it, and task category — aimed at industry-wide transparency.
