While documenting a payments service that is roughly 70% agent-built, the author found a hardcoded skip in the nightly reconcile — one account ID in a list, no comments, no linked ticket, a two-line PR description, merged eight months ago — and nobody currently at the company was part of that decision.
The lesson for teams running agents at scale: assign someone to record why, not just what. Ironically, CodeRabbit's PR summary still accurately describes the skip. Decision-context loss is becoming a new form of technical debt in the AI-generated code era.
