Claude Code v2.1.268 ships gateway billing sync, fixes HTTP 400 on third-party endpoints

anthropics/claude-code released v2.1.268 with notable changes:
- Gateway billing sync: setting pricing: in gateway.yaml makes /cost and telemetry match the spend meter for managed clients
- Security warnings: startup alert when accesscontrol.allowcidrs is empty; new gatewayInternalNetworks managed setting
- New claude self-hosted-runner --remove-session-state; -- flags for plugin commands
- Key fixes: HTTP 400 on every turn for third-party Anthropic-compatible endpoints since 2.1.265 (Artifact tool regex rejected); WebFetch now times out after 300s instead of hanging (override via CLAUDECODEWEBFETCHDEADLINEMS); busy-loop CPU pinning in idle sessions; spurious "message came through empty" after MCP calls; deny/ask rules not applying to symlinked dirs like /etc and /tmp
