Claude Code 2.1.265 ships hot-reloading plugin dirs and prompt-cache fixes

Claude Code CLI 2.1.265 lands with 50 changes. Highlights: --plugin-dir can now point at a folder of plugins, with child manifests auto-loading and live add/remove detection; tool results saved to disk get a 1 GB cap with truncation shown in-conversation; and Desktop/Cowork telemetry via the apps gateway now includes user.groups to match terminal sessions. Key fixes target prompt-cache reuse breakage: resumed foreground-spawned subagents changing tool lists and system prompt prefixes, and agent teammates/resumed subagents pushing SubagentStart hook context and preloaded skills out of the prompt prefix. Also fixes resume after a process died mid-tool-run.
