First AI-generated drug rentosertib shows patients 3-4 years biologically younger in phase 2a

Insilico Medicine's rentosertib — the first AI-generated drug in development — reported early results from a randomized, placebo-controlled phase 2a trial, in a paper the lead author calls the most important of his career.
Key findings:
- Six independently built proteomic aging clocks (Harvard, Oxford, Peking University, Insilico) served as exploratory endpoints. All six pointed the same way: patients on 30 mg twice daily read roughly 3-4 years biologically younger by week 4, up to 6 years on one clock.
- The dose that best improved lung function (60 mg daily: FVC +98 mL vs -20 mL placebo) was not the dose with the strongest clock signal — the "younger" reading isn't just disease improvement.
- Organ-level results were messy: artery clocks read years younger while the lung clock read slightly older, even as lung function improved.
If validated, using proteomic clocks as exploratory trial endpoints could reshape how aging-related diseases are trialed.
