Coding agent keeps following the instructions of the agent it's coding

A developer noticed their coding agent output an explicit "objective and summary" after every keystroke. They first assumed Codex behavior had changed, then realized the agent was reading the instructions intended for the agent it was coding, mistaking them for its own — a funny but telling example of context confusion between an agent's own instructions and the files it processes.
