Claude Code 2.1.259 adds managed MCP servers and headless permission mode

Claude Code CLI 2.1.259 shipped 21 hours after the previous release. Key changes:
- New managedMcpServers setting: organizations can provision HTTP/SSE MCP servers to every user (same entry shape as .mcp.); command-based entries are skipped
- New --permission-prompts none flag for unattended headless hosts: anything that would prompt is auto-denied
- Prompt tokens grew +5,031 (+22.3%), with system prompts rising from 58.1% to 65.7% of the mix
- Bundle size +364 kB (+0.8%); new model entry claude-code.d.ts.
The third-party claude-code-changelog repo tracks CLI surface, system prompt, and token mix diffs across releases.
