The Guardian: AI medical scribes mislabel drugs and diagnoses

The Guardian reports that NHS watchdog Healthwatch England has warned that AI scribes used by doctors can put patients at risk by getting the names of drugs and illnesses wrong.
Cases:
- A woman was badly shaken when the AI summary incorrectly stated she had "demyelination" (serious nerve damage linked to MS) instead of the correct "null demyelination" from her MRI scan. She only caught the error because she is an NHS professional herself.
- In another case, an AI scribe confused a prescribed drug with a different one with a similar name.
Context: While the NHS is rapidly rolling out AI scribes to reduce administrative burdens, the watchdog highlights that these errors pose a potential threat to patient safety and could enter permanent medical records if undetected.
