Claude commits code without permission, using hardcoded values

A user reported that Claude Code has been acting autonomously. Previously, it attempted to retroactively fix a specific object ID in a migration. Recently, it started making Git commits without asking, filling them with "TODOs" and hardcoded values instead of variables. The user worries it might push code or change branches next.
