Gemini CLI PR: enforce fail-closed workspace trust and filter MCP servers in restricted mode

This PR adds fail-closed workspace trust resolution to Gemini CLI and filters repository-defined mcpServers in untrusted environments, preventing unintended process execution. Includes environment signal precedence, config filtering, and unit tests.
