Moderna/Merck cancer vaccine trial succeeds: ML ranks neoantigens for personalized mRNA
zakkohane · x · 2026-08-21
Moderna and Merck reported success in a Phase 3 trial for a personalized mRNA cancer vaccine involving 1,137 patients, where every dose was unique to the patient's tumor profile.
The Pipeline: Surgical resection → Whole exome + RNA sequencing → ML neoantigen ranking → mRNA encoding up to 34 patient-specific targets → Manufactured and shipped in 8 weeks.
The ML Step: The algorithm ingests genomic data to identify somatic mutations and predicts which will be immunogenic—displayed on the tumor cell surface to trigger a T-cell response. It is designed to learn from accumulated clinical and immunogenicity data across patients, not just per-patient optimization. This represents a significant breakthrough for AI in precision medicine.
