A technical diligence scan of a SaaS target revealed 14 AI tools in use, despite the CTO approving only two. Employees had pasted customer PII into personal ChatGPT accounts and built internal workflows using unsanctioned API keys billed to personal credit cards. Security knew about two tools, and Finance knew about zero, highlighting a critical gap in AI governance.
