AI-designed personalized mRNA cancer vaccine succeeds in Phase 3; Moderna stock jumps 110%

Moderna and Merck announced that their personalized mRNA melanoma vaccine intismeran (mRNA-4157) combined with KEYTRUDA met both primary endpoints in the Phase 3 INTerpath-001 trial — the first personalized neoantigen therapy ever to succeed in Phase 3. Moderna shares surged over 110% on the news.
The therapy sequences a patient's tumor and healthy DNA, then uses an AI algorithm to select up to 34 neoantigen targets for a bespoke mRNA vaccine that trains the immune system to hunt cancer cells. Prior Phase 2 five-year follow-up showed a 49% reduction in recurrence or death risk and 59% reduction in distant metastasis or death, with 5-year disease-free survival rising from 49.1% to 68.8%. AI also powers Moderna's Maestro system that orchestrates production scheduling and delivery of each patient's batch.
The INTerpath program spans 9 Phase 2/3 trials across melanoma, NSCLC, bladder and renal cancer, with the NSCLC Phase 3 already recruiting. Full data will be presented at an upcoming medical meeting, with regulatory submissions planned.
