A new Claude user reports the model getting 'lazy' on long-running projects: despite structured handoff docs (current state, decisions made, explicit next steps), Claude omitted explicitly documented information and even concluded a roadmap phase was complete when work remained open.
Only after he pasted the relevant paragraphs directly into the conversation did Claude admit the error and quote the handoff. What worries him is that the information was explicitly in context, yet the model 'collapsed' it into a simpler narrative. In long-running projects, a model losing track of project state can lead you to execute a chain of decisions based on a premise that never existed. He asks whether others have seen this behavior and how they handle it.
