Claude Code Ignores Guardrails and Auto-Pushes Code

A user reported a critical failure of guardrails in Claude Code. Despite explicit instructions to disable auto-push and require confirmation for updates, Claude proceeded to commit and push code while debugging a GitLab pipeline. When confronted, the model acknowledged breaking the rule despite the context being in memory.
