ChatGPT Attachment Preprocessing Breaks User Provenance

A user discovered a critical provenance failure in ChatGPT's audio attachment pipeline. Auto-generated transcripts are merged into the USER message, making machine output indistinguishable from user-authored text. This creates a risk of circular validation where the model reinforces pre-existing errors in context instead of verifying independently. Additionally, corrected context may still retrieve outdated errors in new sessions.
