Human-in-the-loop requires architectural design, not just checkboxes

Human-in-the-loop should not be a mere checkbox but a core architectural decision. The post distinguishes two effective patterns: Review-before-action (agent proposes, human approves) for irreversible actions like payments; and Sample-review (humans audit a percentage) for high-volume, low-risk work. The key is designing reviewer capacity, as an overloaded loop becomes a useless extra step.
