Description: OpenCode crashes on launch if Cloudflare environment variables (e.g., CLOUDFLAREACCOUNTID) exist without CLOUDFLAREAPITOKEN.
Symptoms: TUI fails to start with a generic "Unexpected server error" and no relevant logs in default mode.
Root Cause: An unhandled exception in Provider.list initialization when partial Cloudflare config is detected, causing a fatal crash instead of a graceful fallback.
Expected: Should skip or mark the provider as inactive if credentials are missing, rather than killing the server process.
