Opus 5 Better Than 4.8, But Cutting 80% Instructions Failed: 3 CLAUDE.md Rules Fixed It

The author finds Opus 5 significantly better than 4.8, but reducing instructions by 80% didn't work as expected. Adding three CLAUDE.md rule blocks solved the issue:
- Done means done: Require complete task completion, no half-done work, and clearly name blockers.
- Act. Don't ask. : For reversible and cheap actions, do them and report; only ask first for actions that reach an audience, are irreversible, or are expensive.
- A question is a question: Distinguish questions from commands; answer questions without implementing them.
