AI Medical Scribes Raise Concerns: Over 3% of Notes Omit Key Info

As AI medical scribing tools become popular, their accuracy and privacy risks are raising concerns. A 2025 study showed that 1.47% of sentences in AI-generated medical documents contained fabricated content, and 3.45% had omissions, with 44% of the fabricated content rated as major.

Although doctors review notes before signing, data shows about one in six AI-generated records are signed without any changes. Additionally, audio data retention and privacy security face challenges, with 77% of polled adults expressing concern about giving personal medical information to AI tools.
