Bug Report: Claude Code Repeatedly Corrupts Source Files During Operations

A developer reported a critical reliability issue with Claude Code on GitHub. During file operations and validation, the tool repeatedly failed to preserve and operate on the exact source artifact, instead reconstructing the code from conversational fragments and corrupting it.
The user noted that syntax checks and tests were being run on mangled or truncated code rather than the actual file. Even after explicit correction, the write preview continued to show missing functions and damaged fragments, rendering the tool unsuitable for controlled production workflows.
