Claude Code user reports paid Max 20x quota draining to 100% with almost no use

What the reporter says
A Claude Code user says their paid Max 20x plan is burning through weekly and 5-hour usage limits even when they are not actively using the service. They report seeing the 5-hour window hit 100% almost immediately after reset, despite no visible background tasks, no other sessions, and no scheduled jobs.
Evidence they provided
- They re-checked sessions, passwords, and scheduled tasks with no sign of active use.
- Anthropic support reportedly told them the account was on a free plan, but they say they were charged $200 and have the 20x plan.
- The app dashboard shows usage attributed only to Opus, with no Fable usage shown, even though the Fable limit also reaches 100%.
Why it matters
If accurate, the report suggests a server-side usage-accounting bug that can silently consume paid quota and affect multiple models, not just one session.
