Codex’s auto-approval reviewer intermittently fails open on codex-auto-review
A GitHub issue against openai/codex says Codex Desktop on Windows is intermittently rejecting actions because the automatic approval review step fails with unsupportedvalue when calling Responses-Lite.
- The error says the model is not supported with X-OpenAI-Internal-Codex-Responses-Lite
- When the reviewer fails, the requested action is blocked fail-closed with “This action was rejected due to unacceptable risk”
- The reporter says logs show the failure belongs to the dedicated model=codex-auto-review reviewer turn, not the main user-selected model
- The issue also mentions a similar failure shape during remote compaction
