Codex Diff crashes in VS Code on macOS
A GitHub issue against openai/codex reports that Codex Diff in VS Code on macOS becomes unusable after Codex edits files: opening the Codex Diff tab immediately shows “Oops, an error has occurred.”
- Reproduced on macOS Apple Silicon with VS Code 1.128.0 and openai.chatgpt 26.721.30844
- The reporter says it happens in every repository, including a fresh workspace
- git diff still works, but the extension log shows fs/readFile failing with No such file or directory (os error 2) followed by an editor-diff-page error boundary
