A Cancer Immunology study on CAR-T therapy finds that germline genetic variants can influence both toxicity and treatment response.
- Researchers integrated whole-germline sequencing from lymphoma patients treated with axicabtagene ciloleucel in the ZUMA-1 and ZUMA-7 trials.
- Putative deleterious variants in STXBP2 were enriched among patients with toxicity in ZUMA-1, though that signal did not replicate in ZUMA-7.
- Mechanistically, STXBP2-deficient or variant-expressing T cells increased inflammatory cytokine production and macrophage activation.
- Variants in ADAMTSL3, a regulator in TGF-β signaling, correlated with protection from toxicity in both trials.
- Variants in PTPN22 were strongly associated with greater CAR-T expansion, a key driver of efficacy.
The authors conclude that inherited genetics may help explain why engineered immune cell therapies vary in safety and performance, and could inform future patient selection and treatment design.
