AI SDK adds `firstChunkMs` to fail fast on stalled model streams

The AI SDK now has a firstChunkMs timeout that fails fast when a stream stalls after returning headers.
- The problem: a model can respond with headers quickly and then never emit a content chunk.
- Previously, that would burn the entire step timeout.
- The new setting lets developers separate “time to first token” from the overall step timeout.
