Claude Code v2.1.212 regresses plan mode into prompting for every Bash command

Claude Code v2.1.212 appears to have regressed in plan mode: sessions inside claude agents now prompt for approval on every Bash command, including read-only actions like git log and grep.
The reporter says this did not happen in v2.1.211 and suspects the change came from a fix that was meant to stop plan mode from auto-running file-modifying commands such as touch and rm. They also note that claude -p still auto-approves read-only commands, suggesting the regression is specific to the TUI path.
