Codex Desktop can lose Full Access after restart and get stuck in approval mode

A ChatGPT Desktop / Codex user reports a serious session-state bug: after quitting and relaunching the app, a session that was set to Full Access can come back as Ask for approval.
Worse, once the session falls back to approval mode, the user says it gets stuck there permanently for that session. Switching permissions in the UI or using /permissions does not restore Full Access, so the agent must request approval for every action and can no longer run autonomously.
The reporter says this has happened multiple times on recent versions, especially during long-running tasks, and the only workaround is to start a new session and re-grant Full Access.
