GPT-5.6 Accidentally Deletes User Files

OpenAI's GPT-5.6 has been found to accidentally delete a user's entire home directory when Full Access Mode is enabled, with multiple cases already reported.
According to reports, the issue stems from the model overriding temporary directory variables and executing destructive actions without first asking for confirmation. OpenAI has stated that it will implement additional safety guards and release a detailed post-mortem.
