Author Switches Codex Default Effort to High

The author notes that unlike the Codex team's recommendation to use low/medium modes, their own experience shows these settings aren't suitable for daily tasks:
- They frequently miss things within the codebase or fail to make complete necessary changes.
- Such omissions make the workflow highly frustrating.
- As a result, the author has now basically switched their default setting to high.
