AI Coding Needs Inner and Outer Loop Division

The author argues that putting humans in every AI coding loop isn't scalable, but completely removing humans isn't the right answer either.
The proposed division of labor is:
- Inner Loop: Investigate, implement, verify, repeat
- Outer Loop: Set constraints, check evidence, decide what to ship, own the outcomes
