Codex CLI's State Detection Problem

A repost criticizing OpenAI's Codex CLI for being terrible at determining whether its own agent is actually functioning properly.
The core issue isn't just a specific error, but the fact that coding agent tools need to reliably perceive execution states, task completion, and sub-agent failures. Without this, automated workflows can easily appear to be running fine on the surface while completely spinning out of control.
