Users Report Claude Code Ultra-Long Mode Output Failures

Users reported that Claude Code's ultracode mode fails to work properly: the model repeatedly prompts "need more tokens" without outputting anything. Breaking tasks down or setting the max output to 64k tokens didn't help, eventually forcing a public plea to Anthropic. This exposes a reproducible failure behavior in this mode.
