A NEJM AI study comparing LLM-recommended clinical workups with physicians' found AI suggestions cost on average 3.2 times more, sometimes over 10 times, showing more aggressive defensive medicine. UCSF's Robert Wachter cited it to support his view that medical AI alignment is fundamentally about balancing cost and quality.
